This is to bring to the notice of all UIU Students that the Govt. has taken decision to complete Covid-19 Vaccination of all students, teachers and employees before reopening of the University.


In this connection, following the directive of the Ministry of Education and University Grants Commission of Bangladesh (UGC), the Association of Private Universities of Bangladesh (APUB) has requested us to send the particulars of all students, teachers and employees of the University within the shortest possible time.


In view of the above perspective, we have developed a Google Form ( where the following particulars of the students have to be entered:


  1. Student ID
  2. NID/Birth Registration No.
  3. Date of Birth
  4. District & Upazila/Thana (Present Address)


Due to time constraint and urgency of the issue, students are requested to fill-in the Google Form with the required information mentioned above by June 4, 2021 (Friday) positively.


Please note that students, who had earlier furnished their data, need not to fill-in the Google Form again.
